Then we will create a customized educational needs analysis survey to quantitatively identify the health and wellness status of your staff.

All educational programs are developed after analyzing and synthesizing the survey results. We will formulate metrics to monitor the outcomes and evaluation of the educational program. 4.  Self-Care Essentials For A Healthier Life™: Healthy Living With Chronic Disease -

Self–Management Program utilizes Holistic Self – Care Wellness Model and a Medical Self Care Model approach to help improve the use and quality of health care by empowering individuals to take a more active role in their health and well–being.

The modules and classes in this program will provide individuals/groups essential information to self–manage their chronic conditions and offer motivation and support on their journey toward living a healthier happier life.
Education Modules:
  1. Self–Care Guide – Healthy Living With Cardiovascular Disease: Heart Failure and Hypertension
    The Cardiovascular Management Module addresses the growing concerns of all aspects of cardiac care: prevention, special needs after a cardiac event and chronic needs of individuals with heart failure. Prevention of cardiac disease starts early and in many cases before there is a diagnosis of heart disease. We focus our efforts on individuals with diabetes (high blood sugar), high blood pressure (hypertension) and high cholesterol (hyperlipidemia). We know that by effectively managing these 3 conditions can prevent the serious and often life threatening effects of heart disease.

    For individuals with heart failure, we focus our attention on improving their quality of life. We provide education on diet, weight management and proper use of medication to prevent unwanted hospital visits that prevent you from getting the most out of life.

  2. Self–Care Guide – Healthy Living With Diabetes
    The Diabetes Management Module addresses the growing concerns of living an active, independent healthy life by making a lifelong commitment to careful disease management.

    The following components of diabetes management contribute to a healthy lifestyle: education, monitoring, healthy way of eating, stress management, complications, medication, motivation, activity and overcoming anxiety of self–Insulin administration.

  3. Self–Care Guide – Healthy Living Body, Mind & Soul
    The Body, Mind & Soul Module addresses the growing concerns of living a balanced integrated life with a focus on health promotion which is the science and art of helping people change their lifestyle to move toward a state of optimal health.

    The following components of body, mind & soul contribute to a integrated healthy lifestyle: stress management, exercise, nutrition, lifestyle choices (smoking, drinking, drugs), preventive healthcare, peace of mind, positive beliefs, attitude, belief in a Higher Power, honoring yourself, and your inner knowing, identifying what gives you hope and support.
